DEFECT #1 - SUSPENSION:FRONT:SPRINGS:LEAF SPRING ASSEMBLY:HANGER,BRACKET

Description
Bolts and rivets securing the forward spring hanger bracket to the frame could loosen and eventually break.

Consequence


Corrective Action
Dealer will replace each fastener with a one-half inch diameter bolt and locknut.  In addition, owners will be advised to have bolts checked about once every six months to ensure that they remain secure.

DEFECT #2 - STEERING:GEAR BOX (OTHER THAN RACK AND PINION)

Description
A high stress crack may exist on the left-hand frame rail on the forward control chassis, in the region of the front left-hand hanger.  In addition, the key between the steering gear box output shaft and the universal joint may be loose.

Consequence


Corrective Action
A reinforcement repair kit will be installed to correct the high stress crack problem.  A high strength bolt will also be installed to modify the gear box output shaft.

Vehicle description: school buses.system: structure; left-hand frame rail, and steering; gear box output shaftand universal joint yoke; key.consequences of defect: the stress crack in the frame rail may cause a wobbleor shimmy condition in the front end of the bus.  This could result in loss ofvehicle control and an accident.  The loose key could also cause loss ofsteering control and an accident.note: if wobble or shimmy is noticed during vehicle operation, vehicle speedshould be decreased.  If it persists, discontinue service until the problem iscorrected.  Also, the connection between the steering gear output shaft and theuniversal joint should be checked.  If the key is loose, discontinue serviceuntil corrected.

DEFECT #3 - SEATS

Description
The seat cushions and seat frames on these van buses may have been installed with insufficient clearance between the seats.  These vehicles do not comply with federal motor vehicle safety standard no.  222, school bus passenger seating and crash protection.

Consequence


Corrective Action
The seat clearance will be checked on all involved vehicles and any buses with inadequate clearance will have their seat cushions and frames relocated at no charge.

DEFECT #4 - VISIBILITY:REARVIEW MIRRORS/DEVICES:EXTERIOR

Description
Vehicle does not conform to federal motor vehicle safety standard no.  111, rear view mirrors.  An outside cross-view mirror was not installed at the time of manufacture.

Consequence


Corrective Action
Dealer will install a cross-view mirror at no charge to owner.

Vehicle description: school buses.system: visual; rear view mirrors, exterior.consequences of defect: the lack of an outside cross-view mirror makes it moredifficult for a school bus driver to determine the presence of children in thearea directly in front of the bus prior to putting the vehicle in motion.  Thiscould result in an accident and serious injury.

DEFECT #5 - STRUCTURE:FRAME AND MEMBERS

A crack may develop in the left-hand frame rail in the region of the left hand front spring hanger.

Consequence


Corrective Action
Dealer will inspect frame and reweld to reinforce the left-hand frame rails, if necessary.
