DEFECT #1 - STRUCTURE:BODY:DOOR

Description
The passenger access doors are equipped with differential air motors for the door opening and closing operation.  Due to the size and construction of the doors, the motors that hold the doors closed can be overpowered and the doors can be forced open by a passenger pushing against them.

Consequence
If the vehicle is in motion as the doors are pushed open,the parking brakes will set, stopping the vehicle suddenly.  This could result ininjuries to standing passengers.  If the doors can be pushed open wide enough,a passenger may fall out of the moving vehicle, causing serious injuries.

Corrective Action
An unloader kit will be added to dump the air off of the small cylinder side of the door motor to insure that the doors are positively locked and cannot be forced open.

DEFECT #2 - UNKNOWN OR OTHER

Description
Vehicle description: diesel powered, rubber tire street cars, 24,ooo lbs.  Gross vehicle weight equipped with a mobile-tech, model 3213, wheel chair step lift.  The passive step lift's barrier which keeps a wheel chair and its occupant from rolling off the lift pad fails when hit by a powered wheel chair.

Consequence
Failure of the wheel chair barrier may allow the powered wheelchair to fall off the lift pad, resulting in injuries to its occupant.

Corrective Action
Change the wheelchair restraining barrier so that wheel chairs will not be able to roll over the edge.

DEFECT #3 - STEERING:COLUMN

Description
The welds on a bracket which attaches the tilt- telescoping steering column to the firewall under the dash are defective and could fail.

Consequence
Failure of the attachment bracket can result in a lossof steering control and a possible vehicle accident.

Corrective Action
A new, improved bracket will be installed on all affected units.
